If you have a few minutes, please help Liz Langthorn (mom volunteer with TTBSG, researcher and future doctor whom is starting med school in the fall) with this survey. PLEASE ALSO PERSONALLY E-MAIL OR SHARE THE SURVEY WITH ANYONE WHO HAS GONE THROUGH THIS. *** Thank you for taking the time to fill out this survey on your experience of having a child with ties. The purpose of this survey is to document the experience of moms dealing with ties in their children in the absence of clear treatment guidelines for ties and related breastfeeding complications. If you have more than one child with ties, please fill out the survey for the one whose story you most want to share or the most recent. Feel free to fill out the survey again for additional children with ties, regardless of revision status. If your child had more than one revision, you will have questions related to the initial revision and and in a separate section, questions focusing on additional revisions. Any identifying information provided by you in the survey will not be shared with any person, organization or group. Any identifying information will be removed. Any quotation(s) used will be stripped of identifying information. Future information about the aggregate data and any resulting publication will be available on the Tongue Tie Babies Support Group Facebook page. If you have any questions, please email me, [email protected] Thank you for your participation, Liz Langthorn, MPH |
